Your Appointment

To schedule an appointment, please call our office at (916) 510-5280 or use our online contact form. We recommend arriving 15 minutes early for your first visit to complete paperwork.

What to Bring

  • Photo ID
  • Current Insurance Card
  • List of current medications and dosages
  • Any relevant medical records or test results

Insurance & Billing

We accept a wide range of health insurance plans. Please contact our office or your insurance provider to verify coverage before your appointment. We are happy to answer any billing questions you may have.

Clinic Policies

  • Cancellation Policy: We request at least 24 hours' notice for all cancellations to avoid a cancellation fee. This allows us to offer the appointment time to another patient in need.
  • Prescription Refills: Please allow 48-72 hours for prescription refill requests. You can request refills by calling our office or through your pharmacy.
  • Privacy Policy: We are committed to protecting your health information in compliance with HIPAA regulations.
